ENROLLMENT REQUIREMENTS
All students must be registered by a parent/guardian and provide the documents outlined below.
*Birth Certificate, Baptismal Certificate, or Passport (must show parent's name)
*Current Verification of Residency such as utility bill, rental agreement, mortgage document, or military housing orders (up to 6 months in advance; this does not apply if parent is on the housing waitlist).
*Current Immunization Record. For the most up-to-date requirements, please visit SDUSD Nursing and Wellness / immunization_requirements.
Be sure to also have the Housing Questionnaire and Health Information Exchange Consent Form ready to upload.
